In order to measure the effectiveness of a communication-priming intervention, we investigated the promotion of goals-of-care discussions between clinicians and elderly hospitalized patients with serious conditions.
A randomized, pragmatic clinical trial, comparing a communication-priming intervention for clinicians against standard care, was executed at three U.S. hospitals, part of a single healthcare system—a university hospital, a county hospital, and a community hospital. Hospitalized patients were considered eligible if they were 55 or more years old with one or more of the chronic conditions used in the Dartmouth Atlas study on end-of-life care, or were 80 or older. Patients presenting with either documented goals-of-care discussions or a palliative care consultation between the time of their hospital admission and the screening for eligibility were excluded. Randomization, from April 2020 to March 2021, was stratified according to study site and prior dementia.

The proportion of patients, whose electronic health records explicitly documented goals-of-care discussions within 30 days, served as the primary outcome. A total of 2512 patients were enrolled from the 3918 screened, with a mean age of 717 years (standard deviation 108). Of these, 42% were female. Randomized assignment placed 1255 patients in the intervention group and 1257 in the control group. American Indian or Alaska Native patients comprised 18%, Asian 12%, Black 13%, Hispanic 6%, Native Hawaiian or Pacific Islander 5%, non-Hispanic patients accounted for 93%, and White patients made up 70% of the patient population. Among patients in the intervention group, the proportion of those with electronic health record documentation of goals-of-care discussions within 30 days reached 345% (433 of 1255 patients), contrasting with 304% (382 of 1257 patients) in the usual care group. This difference, adjusted for hospital and dementia factors, was 41% (95% confidence interval, 4% to 78%). A larger intervention effect size was observed among patients with minoritized racial or ethnic backgrounds, as suggested by the examination of treatment effect modifiers. A disparity analysis of 803 patients with minoritized racial or ethnic identities revealed a 102% (95% confidence interval, 40% to 165%) increase in hospital- and dementia-adjusted goals-of-care discussions for the intervention group compared to the usual care group. In the group receiving usual care, compared to the intervention group (comprising 1641 non-Hispanic White patients), the adjusted proportion of goals-of-care discussions was 16% (95% CI, -30% to 62%) lower. No significant difference in the intervention's impact on the primary outcome was detected based on factors like age, sex, dementia history, or the location of the study.

To analyze the link between parental self-reported health and children's economic status in China in 2014, this study employed inverse probability of treatment weighting to address selection and endogeneity bias within nationally representative data. Potential mediating factors in this relationship, which we further examined, include depressive symptoms, social support from family and friends, emotional closeness to children, and financial contributions from children.
Parents whose children enjoyed more financial success were, the study shows, more likely to perceive their own health as being better. Depressive symptoms were the most prominent mediating influence on the well-being of older adults, affecting both rural and urban residents equally. In contrast, the relationship between children's economic situations and perceived health was mediated by the size of support networks only among rural older adults.
Based on this study, it is hypothesized that the financial success of children is associated with enhanced self-perceived health in older generations. The presence of greater emotional well-being and more easily accessible support resources amongst parents in rural areas with successful children partially elucidated this relationship. A quasi-causal examination of the data indicates that adult children continue to hold substantial importance for the well-being of their elderly parents in China, but also implies an exacerbation of health disparities in later life due to the possibility of having economically successful offspring.
